There's a 4 second bit in the opening credits of three tachikomas swerving in reverse on an elevated highway and firing at one of the black long-armed armor suits. Right at the "Save your tears / for the day" lyric.

I don't remember ever seeing this scene in the series - was this animation from an episode never aired, or something drawn specifically for the credits?

All the scenes in the credits were specifically made for them, as far as I can tell. I can't think of a single scene, that would double in the series.

Yes that's true, the endings and openings are all specifically designed for the fit of an opening. No scene is reused. Even though that would be very very neat.
